About us: Acuris Care stands for high-quality care and caters to the individual needs of our clients. In addition to traditional home care services, we also support and employ family caregivers in the Canton of Zurich.

Your role: As a Registered Nurse, you will be responsible for the case management of our clients in their homes. A unique aspect of Acuris Care is the close collaboration with family caregivers who are employed by us. You will serve as a qualified point of contact and ensure the quality of care.

Your responsibilities:

Case management: Needs assessments (RAI-HC), care planning, and evaluation of the care process.
Medical care: Performing complex medical-technical procedures.
Consultation & instruction: Providing professional guidance and support to employed family caregivers (education).
Coordination: Collaboration with general practitioners, hospitals, and external partners.
Quality: Ensuring adherence to guidelines and documentation.

Your Profile

Graduation as a Registered Nurse (RN) (HF or FH) or Swiss Red Cross (SRK) recognition.
Experience in home care and with RAI-HC needs assessments is an advantage.
Driver's license category B (mandatory).
Good written and spoken German skills.
Work style: You work independently and in a structured manner, maintaining a clear overview even in dynamic situations.
Flexibility: We operate on a "give and take" model. We offer flexible working hours, but in return, we expect a willingness to adapt and find solutions when needed, especially during periods of high demand or when client needs change.

Your Responsibilities:

Nursing: Providing basic and therapeutic care in complex and stable care situations.
Medical Technology: Competently performing medical procedures (e.g., wound care, medication management, injections) according to your qualifications.
Documentation: Maintaining meticulous and complete nursing records.
Collaboration: Actively communicating within the team and collaborating with family caregivers on-site.
Monitoring: Identifying changes in health status and reporting them to the case manager.
Nursing Care: Providing basic and therapeutic care in complex and stable care situations.
